	The student intern’s immediate supervisor must complete the form at mid-term and the end of the term.* (Those exact dates are included on the Internship Contract and the intern will know the dates.)
	In each category on the form, circle the number in each item that best reflects the intern’s performance.
	Please add your comments in the area to the right of each item.
	Because this evaluation will be part of the documentation of the student’s internship experience, supervisors must e-mail the completed form to pdixon@ec.edu or mail or FAX the signed/completed form to
